📃Scientific paper: Malignant testicular unclassified sex cord stromal tumor: a case report Abstract: BACKGROUND: Most testicular tumors are germ cell tumors; sex cord stromal tumors are infrequent, accounting for only 3–5% of testicular tumors. Unclassified sex cord stromal tumors are extremely rare. Generally, 10% of sex cord stromal tumors are malignant. We report a case of malignant unclassified sex cord stromal tumor with retroperitoneal lymph node metastasis at first visit and a corresponding literature review. CASE PRESENTATION: A 72-year-old Japanese man visited our department primarily for indolent right scrotum enlargement in September 2020. Blood biochemistry examination, urinalysis, and tumor markers (alpha-fetoprotein, human chorionic gonadotropin, and lactate dehydrogenase) showed no abnormal findings. Contrast-enhanced computed tomography showed enlarged para-aortic lymph node (18 × 16 and 10 × 10(2) mm); a 50 × 45(2) mm mass with uneven contents was found in the right testicle. The patient underwent inguinal orchiectomy in September 2020. As per immunohistochemistry, the tumor cells were diffusely positive for SF-1 and Ki-67, partially positive for inhibin, and negative for CAM5.2, CK7, CK20, C-KIT, CD30, LCA, GATA-3, TTF-1, and PAX8. Calretinin was expressed in approximately 5% of tumor cells; thus, sex cord/gonadal stroma components were considered to be involved. 📃Scientific paper: Sex differences in treatment patterns for non-advanced muscle-invasive bladder cancer: a descriptive analysis of 3484 patients of the Netherlands Cancer Registry Abstract: Purpose Bladder cancer (BC) is a common malignancy with well-established differences in incidence, clinical manifestation and outcomes between men and women. It is unknown to what extent disparities in outcomes are influenced by differences in treatment approaches. This paper describes treatment patterns among men and women with muscle-invasive BC focusing on curative treatment (radical cystectomy or trimodal therapy). Methods A retrospective population-based cohort study was performed with data from the Netherlands Cancer Registry. All patients newly diagnosed with muscle-invasive, non-advanced BC (MIBC, cT2-4a, N0/X, M0/X) in the years 2018, 2019 and 2020 were identified. Patient and tumor characteristics and initial treatment were compared between men and women with descriptive statistics and multivariable logistic regression analyses. Results A total of 3484 patients were diagnosed with non-advanced MIBC in 2018–2020 in the Netherlands, of whom 28% were women. Women had higher T-stage and more often non-urothelial histology. Among all strata of clinical T-stage, women less often received treatment with curative intent (radical cystectomy [RC] or trimodality treatment). Source: Cancers High pretreatment DHEA levels are associated with poor clinical outcomes in patients with metastatic non-small cell lung cancer (NSCLC) treated with immune checkpoint inhibitors (ICIs). A study found that patients with high DHEA levels had significantly shorter progression-free survival (PFS) and overall survival (OS) compared to those with low DHEA levels. This suggests that sex hormone levels may influence the response to ICIs in NSCLC patients.

In continuation of the previous post... 6. Lets now focus on the prostate. The prostate is a male sex gland. It is a size of a walnut. It is located in pelvis, anterior to the rectum, behind the base of the penis, and inferior to the the bladder. It surrounds the urethra and it’s main function is seminal fluid production. The seminal fluid aids in transportation, protection and nutrition of sperm cells. The sperm cells are produced by the testis. 7. Prostate cancer is a serious disease. Fortunately, most people with prostate cancer get diagnosed before it spreads beyond their prostate gland. Treatment at this stage often eliminates the cancer. Some studies have shown that men with low ejaculation frequency are at higher risk of prostate cancer. 8. A man is diagnosed with prostate cancer every 2 minutes; 1 in 8 men will be diagnosed with prostate cancer during his lifetime. Prostate cancer is the second leading cause of cancer death in men. If detected early, the survival rate is nearly 100%; however, the survival rate for advanced prostate cancer drops to 32%. Black men are 2 times more likely to be diagnosed and likely die as a result of prostate cancer than white men; 1 in 6 black men will be diagnosed with prostate cancer during their lifetime. 9. There are often no symptoms during the early stages of prostate cancer, but screening can detect changes that may indicate cancer. A test that measures levels of Prostate Specific Antigen (PSA) in the blood. High levels suggest that cancer may be present. However symptoms could be: a. Difficulty starting and maintaining urination b. A frequent urge to urinate especially at night c. A weak urine stream d. Blood in the urine or semen e. Painful urination or ejaculation f. Pain in the back hips or pelvis g. Urinary incontinence 10. Not all growths in your prostate are cancer. Other conditions that show symptoms similar to prostate cancer include: a. Benign Prostatic hyperplasia (BPH): At some point, some men will develop BPH. This condition enlarges the prostate gland but doesn’t increase the cancer risk. b. Prostatitis: In men younger than 50 years, an enlarged prostate gland is most likely prostatitis. Prostatitis is a benign condition that causes inflammation and swelling in the prostate gland. Bacterial infections are often the cause. 11. Prevention of the disease condition are not limited to the following: a. Maintaining a healthy weight b. Exercising regularly, it is recommended that we participate in about 150 minutes of moderate-intensity exercise every week or a little more than 20 minutes daily. c. Consuming a nutritious diet. We should continue to eat fruits, vegetables and whole grains. Avoid red meats and processed foods. d. For those that smoking, they are advised to quit. e. Hello members A very nice case has been diagnosed : NON HODGKINS LYMPHOMA. I know each and everyone is aware of it , but this is very different case and it took 6 hours to reach the final conclusion , that how it happens. So: A 24 years old unmarried , beautiful looking with shiny eyes girl / women came with a history of abdominal pain since 2 weeks. On detailed clinical history and physical examination in presence of female staff we found tenderness in right hypochondrial region. The patient also gave a history of extraction of tooth due to loosening while she noticed when she was brushing her teeth's in morning. USG scan was done and it revealed only mild hepatomegaly and all other parameters are withnin normal range. The overlying mucosa was reddish pink. The swelling was sessile with ill-defined borders, reddish pink in color and was firm in consistency as stated by dentist. Routine hemogram analysis, urine analysis, and X ray chest were normal. The patient was negative for HIV and hepatitis B virus. There is no need to go for further investigations as i thought everything is normal. Though , she was very fatty in weight , so I just asked casually "WHY YOU PUT ON SO MUCH WEIGHT: , as I thought it was some thyroid related issues but hormonal assay was turned out to be normal. As stated by her she was very fond of "BREAD & CHEESE WITH ROASTED TYPE". Also there is expansion of the malar bone on the right side laterally with involvement of middle and inferior nasal conchae medially with presence of swelling over maxillary antral bone. Biopsy was done which revealed uniform monotonous proliferation of small round cells with prominent nuclei , associated with necrosis going in suspicion of ? lymphoproliferative disease. Immunohistochemistry shows positivity for CD45. The final diagnosis was given as Non-Hodgkins lymphoma , based on FISH analysis , but i want to tell all of you that how it happened since every parameters were normal. As the patient is very fond of cheese and bread as stated by dietician, so these products contains estrogens and insulin like growth factor 1, so she is fond of eating 25 slices per day as it leads to excess consumption which in turn causes mutation in TNF and interleukin 10 genes. THAT'S WHY THIS WAS A RARE CASE OF NHL RELATED TO CONSUMPTION OF EXCESS BREAD AND CHEESE , SO WE NAMED THE WOMEN " A CHEESY GIRL". It's a very rare case of NHL we reported linked with consumption of excess amount of cheese.
