Testicular fibroma of gonadal stromal origin is a rare benign tumor of testis which usually presents as a slow growing testicular mass. Only 25 cases of testicular fibroma have been reported in the literature. Presence of minor sex cord elements in this tumor is even rarer. We report a case of testicular fibroma with minor sex cord elements that involved almost the entire testis and tunica vaginalis. The patient presented with hydrocele, a rare presentation for this entity. The rarity of the diagnosis and the clinical presentation prompted this case report.

Testicular (gonadal stromal) fibroma: Case report and review of the literature2002 •
A 25-year-old man presented with complaint of a painless enlargement in his left testis. The solid, encapsulated, circumscribed and grayish-white testicular mass displayed the characteristics of testicular fibroma histologically. It was composed of acellular collagenized plaques and hypercellular areas of fibroblastic spindle cells. Immunohistochemically, the neoplastic cells were positive for vimentin and smooth muscle actin, but not for cytokeratin, S-100 protein, desmin, CD99/MIC2 (a protein expressed by Sertoli cells and granulosa cells) and CD34. Only 18 cases of testicular (gonadal stromal) fibroma composed exclusively of spindle cells have been reported to date. An additional case of fibroma, which lacks definite neoplastic sex cord elements, and its differential diagnosis from other mesenchymal lesions of testis are discussed here, together with other cases in the literature.

Background: Pattern of a disease is studied with the idea of getting information about the clinical presentation, the morphology and the causative factors. The testicular tumors constitute 4 th most common cause of death from neoplasia in younger males. Though the etiology of testicular cancers is not well understood, various factors like cryptorchidism, trauma infections and genetics play role in the disease. The testicular tumors are histologically diverse. Aim: Our study was undertaken to study the clinical presentation and morphological patterns of the testicular tumors. A total number of 50 cases of testicular tumors were studied. The majority of the cases i.e. 48 (96%) were of germ cell origin. Only 2 cases (4%) of testicular lymphoma were reported. Materials and Methods: The study was carried out in two parts – retrospective (2005-2007) and prospective (2008-2010). During this period a total number of 50 cases of testicular tumors were diagnosed on small biopsies and orchidectomy specimens. Results: The study work comprised of 50 cases of testicular tumors among which 44 (88%) cases were malignant and 6 (12%) were benign (Table 1). The majority of the testicular tumors i.e. 48 cases (96%) were of germ cell origin. Only 2 cases (4%) of testicular lymphoma were reported (Table 2). No case of sex cord stromal tumor, mixed germ cell sex cord tumor and metastatic tumor was reported. Conclusion: Tumors of the testes present a great variety of histological types with many structural variations. The present study fairly provides an insight into the clinical presentations, prevalence and patterns of testicular tumors.

Update on the Pathology of Testicular TumorsAt least 90% of testicular tumors belong to the group of germ cell tumors (GCTs), which are classified according to the 2004 World Health Organization (WHO) classification. Race is one of the most important etiologic factors in the development of GCTs. White men living in Western industrialized countries show the highest rates of incidence. Known risk factors are cryptorchidism, contralateral GCT, familial association, infertility, testicular atrophy, trauma, surgery, socioeconomic status, environmental factors, and occupational exposure to noxious conditions. For the most part, the morphology of GCTs is well known to pathologists. There are, however, some little-known rare entities like anaplastic type of spermatocytic seminoma. In the group of nonseminomatous GCTs are emerging the somatic-type malignancies (carcinomas, sarcomas) arising in teratomas. Tumors of sex cord/gonadal stroma account for 1.6-6% of adult testicular tumors and are somewhat more frequent in children. Absolutely nothing is known about the epidemiology, histogenesis, and possible etiology of these tumors, which derive from Leydig, Sertoli, granulosa, and theca cells. In the group of "miscellaneous tumors," lymphomas are the most frequent testicular tumors in men older than age 50.
