A 51-year-old man presented with a 2-year history of painless lump in the left hemiscrotum. Scrotal examination demonstrated a 3-cm, firm-to-hard mass attached to the left spermatic cord. Scrotal ultrasound scan revealed a heterogeneous mass separate from the left testis and epididymis. An orchidectomy was recommended to the patient; however, he declined surgery and underwent excision biopsy of the lesion with preservation of the left testis. Histology suggested a leiomyosarcoma of the spermatic cord and positive margins. Following a normal staging CT scan, the patient was referred to the regional sarcoma clinic. He was treated with an orchidectomy. Microscopic examinations showed fibrosis in the spermatic cord and negative margins. The patient has been followed up for 12 months with CT scans and shows no signs of recurrence.

Leiomyosarcoma of the spermatic cord with scalp metastasis: case report and literature review2014 •
Herein we present 82-year-old man with leiomyosarcoma arising from the spermatic cord with scalp metastasis, five years after primary surgical treatment. Complete surgical excision is required in such cases, as well as precise evaluation of further therapy. Paratesticular leiomyosarcoma is a rare entity, malignant mesenchimal tumor of smooth muscle differentiation. Although leiomyosarcomas of different localizations have well-known metastatic potential, cutaneous metastases are extremely rare with only 16 cases described in the literature. To our knowledge there are no reported cases of the paratesticular leiomyosarcoma metastatic to the skin. This article reviews the literature regarding paratesticular leiomyosarcoma presentation, diagnosis and treatment.

Radiologic Pathologic Correlation of Primary Paratesticular RhabdomyosarcomaRhabdomyosarcoma is the most common soft tissue sarcoma in childhood and adolescence, arising from the mesenchymal tissue. It is an intrascrotal tumor localized in the paratesticular structures such as the epididymis or spermatic cord. The majority of this tumor occurs in the first two decades of life. Ultrasound is considered the imaging modality of choice for evaluating an intra-scrotal abnormality. We present a case of a 4-year-old boy with a 2-weeks history of rapidly increasing left scrotal swelling which was clinically diagnosed as hydrocoele. We present the ultrasound, elastography and MRI features of a case of paratesticular embryonal rhabdomyosarcoma with pathologic correlation and also emphasize need to consider sarcoma as a differential diagnosis for rapidly growing scrotal swelling.

Epidydimal leiomyosarcoma: a rare case of scrotal swellingSarcomas of the genitourinary tract are extremely uncommon and accounts for only 1-2% of genito urinary malignancies. Sarcomas of the para testicular region, comprising tissues such as the epididymis, spermatic cord, inguinal canal and testicular tunica are also extremely rare. epidydimal leiomyosarcoma accounted only for 4 percentage of all para testicular tumours and only 16 cases are reported so far in literature and they account 4% of all Para testicular sarcomas. We are presenting a 61-year-old patient presented with a hard welling of 1 year duration, with no other associated symptoms. On ultrasound evaluation, it was reported as extra testicular lesion, possibly from epididymal tail. We performed a high inguinal orchidectomy. Histopathological examination revealed a para testicular leiomyosarcoma arising from epididymis. This case has discussed because of the rarity of the disease and possible cure if diagnosed early and treated aggressively.

Primary Osteosarcoma of Testis and Para-Testicular Tissues: A Review and Update of the Literature2019 •
It has been stated that genitourinary sarcomas are not common in adults and that they had been estimated at less than 2.7% of all sarcomas. [1-2] It had also been iterated that extra-osseous paratesticular soft tissue osteosarcoma is an extremely uncommon type of malignant tumor. Hong R, et al. [3] stated that due to the rarity of para-testicular osteosarcoma; its histogenesis; treatment; and as well as its specific survival rates were not available. Osteosarcoma of the testis refers to a testicular tumor that is composed of pure osteoid forming malignant cells with no skeletal origin. Gordetsky J [4] Due to the extreme rarity of primary and metastatic osteosarcomas of the testis and the scrotal contents; it would be envisaged that majority of clinicians globally would not be familiar with the presentation; diagnosis; treatment and biological behaviour of the disease. The ensuing article on primary osteosarcoma of testis and para-testicular tissues: a review and update of the Literature is divided into two parts: (A) Overview and (B) Miscellaneous narrations; summations; and discussions related to case reports; case series; and studies related to osteosarcomas of the testis and para-testicular tissues. Strangulated Inguinal Hernia Accompanied With Paratesticular Leiomyosarcom2018 •
An 87-year-old man with dementia was referred to the orthopaedic clinic of our hospital, suffering from spinal symptoms. Clinical examination revealed swelling of the left hemiscrotum and a painless ipsilateral palpable scrotal mass, which was attributed to a possible inguinal hernia. The patient, who was unable to provide details regarding the time of appearance of the mass, subsequently underwent an ultrasound scan of the scrotum (Figs. 1-3). Based on the imaging findings surgical excision was decided (Fig. 4).

sarcomas are rare variety of malignant tumours arising from mesodermal cells. The paratetsticular sarcomas constitute major proportion of primary tumours of paratesticular area. These tumours often pose diagnostic difficulty and it is wise to consider histological examination before going for surgery.
